Case Management is a collaborative approach to provide and coordinate health care services. Our Case Management Coordinators (CMC) work with the patient, family, physicians, center staff and other health care providers to ensure the patient receives all the necessary services to promote healing while planning for a safe discharge.
The CMC bridges the gap to assure that the patient and family has all the information needed to make well-informed health care decisions. This includes helping with understanding and coping with the medical, insurance, and emotional issues confronting an in-patient stay. A comprehensive assessment is performed and recommendations to enhance the patient's health care and living situation are provided. The CMC creates a clear plan to coordinate the healthcare needs while maintaining or improving the patient's optimal recovery. The CMC is with the patient through every step of the recovery process to insure the patient’s needs are met.
The CMC's primary customer is the patient, followed by the patient's family, the center's interdisciplinary team, the physician, the hospital discharge planners, and the community resources to be used by the patient. It is the CMC's responsibility to manage and coordinate the needs and expectations of all of the parties involved.
We know that Case Management improves customer satisfaction and improves patient outcomes. We are one of the few organizations that advocate for a true case management model to coordinate the needs of the patient.
